Dental Health Program


Through a unique service agreement with KFL&A Public Health, Kingston Community Health Centres is pleased to offer a dental health program at its 263 Weller Avenue location. Our goal is to provide accessible dental health care for children and adults in financial need.

Who is eligible: Effective January 1, 2016, we welcome patients who qualify through the new integrated Healthy Smiles Ontario program that expands access to dental services to more children and youth from low-income families. The new program unifies the dental health coverage from the following programs:

Ontario Works
Temporary Care Assistance
Ontario Disability Support Program
Assistance for Children with Severe Disabilities program
Emergency and Essential Services Stream

Children and youth currently receiving care from the programs listed above are still eligible for free dental services. They will be automatically transitioned into the new Healthy Smiles Ontario program.

Healthy Smiles Ontario (HSO) – Eligibility requirements:
Be 17 years of age or under; be a resident of Ontario; not have access to any form of dental coverage (including Ontario Works, Ontario Disability Support Program); come from a household with an income that is at or under the levels shown in the table below, plus $1,670 per additional dependent child.

How to enrol

You can enrol in the program online or by mail.

Once your child’s enrolment is confirmed, you will get a Healthy Smiles Ontario dental card in the mail. You must present your card to your dental provider at each visit to receive services under the program.

Before you start

Before you begin the online application, you need:

  • your child(ren)’s date(s) of birth
  • a valid Ontario address
  • your Social Insurance Number (SIN) or Temporary Taxation Number (TTN)
  • to have filed your taxes in the previous year
  • to be ready to print

Once you submit the online application, you will need to print, sign and mail a Consent Form, within 30 days, to:

Adults

This program may benefit:

  • Adults with a household income under $35,000 and without private healthcare coverage
  • Those in receipt of Ontario Works or Ontario Disability Support Program benefits
  • Those in receipt of Non-Insured Health Benefits for First Nations & Inuit
